I do hereby assume full responsibility for any damages, injuries (including death), or losses that I may sustain or incur, if any, while attending, engaging, practicing, participating or witnessing activity and/or certain event(s) occurring in or about the premises or at any offsite location. I hereby assume full risk, waive all claims and release and hold the Diamond Warriors, individually or otherwise, harmless for any and all liability, claims, suits, damages, expenses, fees, actions, or rights of action or judgements as a result or death to myself or members of my family or heirs, or my guests, or damage, destruction or loss to my property, which in any way relates to, arises out of, or is in any way connected with my presence on the premises, or my participation in events of activities thereon, or the negligent acts or omissions of the releases or any other third party.

I agree to wear all protective equipment required while participating in the activity, and I am fully aware and understand that the Diamond Warriors does not have on or about the premises, or employ or contract with any medical services, provisions for ordinary or emergency medical services.

As legal guardian of the aforementioned person, I hereby agree to individually provide for the possible future medical expenses which may be incurred by my child as a result of any injury sustained while participating with Diamond Warriors and/or any facility used by Diamond Warriors. In case of an emergency, I authorize Diamond Warriors to administer first aid to my child/or take my child to a physician or hospital for further treatment. This acknowledgement of risk and waiver of liability, having been read thoroughly and understood completely, is signed voluntarily as to its content and intent. I represent that I have adequate insurance to cover any injury or damage I may suffer or cause while participating in this activity, or else I agree to bear the costs of such injury or damage myself. I further represent that I have no medical or physical condition which could interfere with my safety in this activity, or else I am willing to assume and bear the costs of – all risks that may be created, directly or indirectly, by any such condition. In consideration of my participation in and the use of Okauchee Lions premises or facilities, I hereby release and covenant not to sue the owner of the premises (releases), shareholders, directors, officers, members, representatives, agents, affiliates and lessees from any and all claims resulting from any physical injury that may occur to me while participating in any program or event sponsored by the Diamond Warriors.

For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby grant Diamond Warriors permission to use the participant’s likeness in photograph/video in any and all of its publications, including but not limited to all of Diamond Warriors printed and digital publications. I understand and agree that any photograph/video using the participant’s likeness will become property of Diamond Warriors and will not be returned. I acknowledge that since my participation with Diamond Warriors is involuntary, I will receive no financial compensation. I hereby irrevocably authorize Diamond Warriors to edit, alter, copy, exhibit, publish or distribute this photo/video for purposes of publicizing Diamond Warriors programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein the participant’s likeness appears. Additionally, I waive any right to royalties or other compensation arising out of, or related to, the use of the photograph/video. I hereby hold harmless and release and forever discharge Diamond Warriors from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND FULLY UNDERSTAND THE ABOVE RELEASE/WAIVER AND FULLY UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING THIS WAIVER VOLUNTARILY.
